Fenwick Vankoesveld test

Definition

A procedure for identifying the cause of a defect in a process that involves asking three times who the originator was of the defect. The Fenwick Vankoesveld test assumes that there can be only one originator of a defect, and is generally carried out by an external auditing group to prevent bias.
