affidavit
Definition
Written and signed declaration of facts made by an entity (called affiant or deponent), voluntarily and under oath, before a notary public or commissioner for oaths. Unlike a statutory declaration, an affidavit is admitted as evidence by courts in place of a witness appearing in person.
